What is a Community Nurse?
A community nurse helps provide supportive services to members of the Enfield community, including:
- Care management and coordination with health care providers, social services, and community resources
- Making home visits to assess health care needs
- Assistance with telehealth appointments
- Health education, symptom management, fall and safety assessments
- Serving as an advocate for medical needs
- Medication education, compliance and monitoring
