Update - COVID-19 Vaccination available in Mascoma Valley

This round has reached capacity.  People who have signed up and are able to be set up with an appointment will be contacted directly.  Please do not reach out to the Town of Enfield for information as we do not have it available.  We will provide any update information as it becomes available.

The Upper Valley Health Council and Dartmouth have partnered with the Town of Canaan and Mascoma Community Health Care Center to offer local COVID-19 shots for residents of the 5 Mascoma Towns (Canaan, Enfield, Dorchester, Grafton & Orange).  This is available to residents 65 years of age and older.  Appointments will be offered BY INVITATION ONLY.  Anyone who qualifies and wishes to participate is highly encouraged to sign up this weekend.   We are not able to provide the original email about this event due to the format, however the text is provided below   (graphics excluded). The Upper Valley Public Health Network is pleased to announce a COVID-19 mobile vaccination clinic in your area on Monday, March 1, 2021.

This Covid-19 mobile vaccination is sponsored by Mascoma Community Health Center, Canaan Fire, Canaan Emergency Medical Service (Ambulance), and Canaan Police. This service is co-sponsored by the Towns of Canaan, Dorchester, Enfield, Grafton and Orange and is open just to the residents of these towns. The invitee list is on a HIPAA secure Town of Canaan website, and it can be used by all residents of all the towns.
 

We need your help to make this a successful event. Some important details are as follows:

Appointments will be offered BY INVITATION ONLY.
MUST be NH residents 65 years of age and older from the Towns of Canaan, Dorchester, Enfield, Grafton and Orange.
This is for people who haven't received a 1st dose of vaccine yet.
The Team CANNOT provide people with a 2nd dose of vaccine at this event.
The vaccination team will return within the required time frame to provide 2nd doses of the vaccine and people will leave this event with an appointment card for the 2nd dose.
It is OK if someone already has a 1st dose appointment through the NH VAMS system and wishes to reschedule with us. Once you are all set for this event, we will instruct you (helping if necessary) on how to cancel your VAMS appointment.

Cancer
Chronic Kidney Disease

Obesity
Pregnancy
Sickle Cell Disease
Type 2 Diabetes Mellitus
Any other serious health issue that affects you.


This is being collected by a HIPAA secure website maintained by the Town of Canaan and will only be used by Upper Valley Public Health Council and/or DHMC to schedule appointments.  

If there is sufficient demand, additional clinics will be scheduled.
